Woman Feels that the System Failed Her Husband, Their Kids, and Her

My husband was put on oxygen and sent home to only be admitted two days later on a BiPap machine.  He was rarely able to remove his mask to eat, was not cleaned well and many times I came to the hospital after working my Medical Assistant shift at a busy Pediatrics’ office only to have to clean him and change soiled bedding.

I feel as if the system failed him, our kids and myself.  I have been unable to obtain his medical records despite requesting them numerous times.  I have supplied our marriage license, his death certificate and both of our drivers’ licenses and still 1-1/2 years later, I still don't have them. I feel like there is something they are hiding.
